If a Tampa Bay assisted living facility, nursing home, or home health agency provides unsafe or substandard care, you can file a complaint with the Florida Agency for Health Care Administration (AHCA), which licenses and inspects them.
How to file
File online through AHCA's complaint portal, by phone via the AHCA Consumer Complaint hotline, or by mail. You can file anonymously, though providing contact information helps investigators follow up.
Document dates, names, what happened, and any photos or records before filing.
What happens next
AHCA reviews every complaint and may conduct an unannounced on-site inspection. Findings and any resulting deficiencies or sanctions become part of the facility's public record on FloridaHealthFinder.
Serious issues involving abuse, neglect, or exploitation should ALSO be reported to the Florida Abuse Hotline (1-800-962-2873).
Check a facility's record first
Before and after filing, look up the facility's inspection and complaint history at quality.healthfinder.fl.gov — a pattern of repeat deficiencies is a meaningful warning sign.
